投標人應具備的資格或業(yè)績(jì):投標人資格和業(yè)績(jì)要求：Qualification and Achievement requirements for the bidder投標人需提供以下文件The following documents must be provided*（1）投標人須按招標文件第一冊第四章格式IV-9.資格證明文件格式的要求填寫(xiě)和提交有關(guān)資料。*（2）投標產(chǎn)品原產(chǎn)地在關(guān)境外的投標人必須是制造商；或者是制造商直接授權的代理商（進(jìn)口產(chǎn)品開(kāi)標前未進(jìn)入中國關(guān)境內的，投標人必須是在關(guān)境外注冊的公司；進(jìn)口產(chǎn)品開(kāi)標前已進(jìn)入中國關(guān)境內的，投標人必須是在關(guān)境內注冊的公司），代理商投標需具備以下條件：①必須提供制造商對本次投標的唯一正式授權書(shū)（授權書(shū)必須由制造商直接授權，不允許二級授權）；授權書(shū)中應包括“制造商對產(chǎn)品及售后服務(wù)質(zhì)量負責”的相關(guān)內容和制造商聯(lián)系人及聯(lián)系方式，并確保制造商聯(lián)系人能夠協(xié)調處理項目執行過(guò)程中的重要事項。（對于此要求，制造商也可在授權書(shū)外另行出具書(shū)面承諾/說(shuō)明），否則不接受代理投標；②投標人在當地依法登記注冊的時(shí)間距開(kāi)標日滿(mǎn)三年，需提供營(yíng)業(yè)執照等注冊登記的有效證件復印件；③距開(kāi)標日三年內，投標人須具有同類(lèi)產(chǎn)品至少兩個(gè)不同年份的銷(xiāo)售業(yè)績(jì)，需提供證明其銷(xiāo)售業(yè)績(jì)的合同或訂單復印件（至少提供2份不同年度的合同或訂單復印件）或用戶(hù)證明，并由投標人代表簽字或蓋章。*（3）投標產(chǎn)品原產(chǎn)地在關(guān)境內的投標人必須是制造商（不接受代理商投標）。*（4）投標人未被中國石************有限公司投標人失信行為管理辦法》的規定，失信積分達到8分時(shí)，暫停失信行為人投標資格半年；失信積分達到9分時(shí)，暫停失信行為人投標資格一年；失信積分達到10分時(shí)，暫停失信行為人投標資格兩年。暫停期滿(mǎn)后恢復投標資格，有效期內的失信記******************集團公司供應商、承包商、******************集團公司機電辦處理******銀行在開(kāi)標之日前三個(gè)月內開(kāi)具的資信證明原件或該原件的復印件（由投標人代表簽字或蓋章）。*（8）第八章中要求的其他合格的資格證書(shū)。(1) All Bidders are requested to fill in according to the requirements of Format IV-9 in Section IV ?and provide relevant information；(2) The bidder whose product origin is outside the customs must be the manufacturer; Or the agent directly authorized by the manufacturer. Copies of the contract / purchase order evidencing its sales performance or Certifications from Enduser should be provided and signed or sealed by the Bidder;(3)The bidder whose product origin is within the territory of the Customs must be the manufacturer (agent bidding is not accepted)(4) The bidder has not been suspended or cancelled the bidding qualification by petrochina Tendering and Bidding website (******). (According to the provisions of the Measures for the Administration of Dishonest Behavior of Bidders of China National Petroleum Corporation Limited, the bidder's bidding qualification shall be suspended for half a year if the dishonest score reaches 8 points; The bidder's bidding qualification shall be suspended for one year if the breach of trust score reaches 9 points; The bidder's qualification for bidding is suspended for two years after the breach of trust score reaches 10 points. If the breach of trust score is 10.5 points or above, or the breach of trust occurs again during the suspension period, the bidding qualification within the scope of the group company will be cancelled for three years. If the bidder is qualified again at the expiration of the term, the breach of trust will not be cleared and the validity period will be extended for three years.)
